Dr. Randolph Bell (Bruce Greenwood) notices that his hand is trembling once more. Devon tries to focus on his grant proposal when he is summoned to the emergency room. AJ leaves his groupie’s room after receiving an alarm that his mother is having trouble breathing. When an automobile pileup occurs, all hands are on deck. Conrad summons Trevor to stitch up one of the patients, but she informs them that a bike rider swerved into traffic and that the rider must be suffering from some sort of illness. Wyatt considers himself fortunate. But when he’s chatting to Conrad and Trevor, he goes out again. Britt, a cartoonist, is checked by Devon and Leela. Leela is quite good at catching a benign tumor. Bell is still concerned about her hand, so she consults with her. They decide not to operate. Before she scrubs in on an operation, Leela tries to guide Britt through her decision about whether or not she should have surgery. Because of his Sarcoid, Wyatt has a heart block. He passed out and fell off his bike. He’ll require a pacemaker.
Wyatt doesn’t want the pacemaker and is ready to gamble that he won’t die if he doesn’t get it. Devon is unable to complete his grant application on time because Kit instructs him to interview candidates for the ER to replace him. Kit, who is overworked and stressed from pulling all-nighters, is thinking about attending Padma’s healing retreat. Trevor and Conrad must persuade Wyatt to get the pacemaker, while AJ must make a television appearance while also keeping an eye on his mother, who is deteriorating. When Leela returns to check on her, Britt, her patient, isn’t there. Devon’s first candidate has a clear plan for leveraging healthcare in the ER and claims to be able to walk Devon through it in an hour or two. Leela approaches Bell for assistance in reaching a choice regarding the patient. Leela decides to have the procedure done by her patient. Trevor is unhappy by the shrink’s decision that Wyatt is competent, and he wants Conrad to intervene. Wyatt’s condition continues to deteriorate. Trevor continually suggesting things that would infringe on his rights in order to save him. Bell isn’t feeling well and has to leave Leela to perform the surgery alone because he isn’t feeling good. This frightens her. She does, however, succeed. Padma’s medical team meets with her for wellness, but they are all called away. Trevor watches as Conrad tries one final time to argue with Wyatt, but they just sit there with him until he dies. Conrad reminds Trevor of how tough it is to do the right thing, and both are emotional. Devon tells Kit that all of the interviewees are burned out, and that he is as well. Conrad tells Bell that he’s not sure if he made the right decision with Wyatt, but that Trevor has learnt something as a result of it. Bell informs Conrad that something is amiss with him and that he needs Conrad to figure out what it is while remaining undetected.
